Why This Decision Is Harder Than It Looks
Choosing a transplant center is one of the most consequential medical decisions a patient or family will make. The publicly available data, survival rates, volumes, waitlist statistics, captures only part of what matters. Two centers with identical survival rates may offer very different patient experiences, support services, and care approaches.

Factor 1: Transplant Volume
Volume is one of the most reliable indicators of center quality. Research consistently shows higher-volume centers tend to have better outcomes, driven by the experience effect and institutional resources (dedicated transplant ICUs, 24/7 coverage, established protocols).
What to look for: Centers performing 50+ transplants per year in your organ type are generally high-volume. Below 20 per year means inherently more variable outcomes.
What it doesn't tell you: Volume does not guarantee every surgeon on staff is experienced. It also does not capture program trends, a center losing volume may be losing staff.
Factor 2: Survival Rates (Read Carefully)
Survival rates on PlainTransplant are risk-adjusted by SRTR. The key comparison is how the center performs relative to its expected rate. A center with 93% that was expected to achieve 90% is outperforming. A center with 97% expected to achieve 98% is underperforming.

Volume vs. survival, kidney programs
Each dot is one of the 40 largest kidney programs by transplant volume. The crosshair splits them at the MEDIAN volume (1170.5/yr) and median 1-yr survival (95.3%) across this set, not the simple min/max midpoint, so each quadrant holds a real share of the data. A high-volume, high-survival program (top right) is the "proven track record" this guide's Factor 1 and Factor 2 describe together.

Position = transplants per year (right is more) and 1-yr survival (up is higher). Color = which of the four zones the program falls in relative to the median split.
Factor 3: Wait Times and Access
Wait times vary enormously. Kidney wait times range from 2 years at some centers to 7+ years at others, driven by local organ supply, listed candidates, and donor acceptance criteria. Centers more aggressive in accepting marginal donors may have shorter waits but different outcome profiles. This is a legitimate trade-off to discuss with your team.
Factor 4: Geographic Proximity
When an organ becomes available, you must be reachable within hours. For heart and lung transplants: 2-3 hours. Kidney has more flexibility but you still need to arrive quickly. Living far from your center creates real logistical challenges during the waiting period.
What No Database Can Tell You
The most important factors in transplant care quality are often unmeasurable:
- Post-transplant follow-up quality: How closely does the center monitor you?
- Multidisciplinary team: Social workers, financial counselors, dietitians, mental health?
- Communication: How responsive is the coordinator? Can you reach someone 24/7?
- Condition-specific experience: Re-transplants, highly sensitized, combined organs?
